Services include, but are not limited to, the following:

  • Assistance and supervision with daily activities such as cooking, eating, dressing, bathing, grooming and toileting
  • Help with activities such as shopping, driving, housework, paying bills and laundry
  • Accompaniment to doctor appointments and any other travel
  • Ensuring that clients takes their medication
  • Socialization and companionship
  • Assistance while traveling – take out

Geriatric Care Management focuses on a client’s entire well being – physical, mental and emotional. We take an integrated approach to care, which includes ongoing Assessments, Evaluations, Monitoring, and Care Coordination.

Assessments

Services begin when you are assigned to a Geriatric Care Manager ( GCM) who is trained to organize and manage your specific needs. They will personally follow your case throughout the entire time of service. The GCM will do an initial assessment based on the physician’s Plan and recommendations, as well as client and family needs in order to create an individual Plan of Care. Then the GCM will instruct and supervise the assigned Caregiver on the necessary care.

Our GCM will provide follow-up assessments on a regular basis and maintain constant communication with the family and agency.

Care Coordination

Our Geriatric Care Managers are responsible for managing and coordinating various services that may be involved in the client’s care that include:

  • Constant communication with the client and their family
  • Scheduling home visits
  • Arrangement for the delivery of medical equipment and supplies (i.e. wheelchairs, commodes, walkers, hospital beds and diapers)
